- The online application for the 2017 award opens January 1, 2017, with an application deadline of April 30, 2017. You must submit your application on-line. To access the on-line application, click here to visit the Louisiana Award System — if the deadline has not expired, you will see a "J.R. Justice On-Line Application" button in the menu on the left side of the web page.
- Upon receipt of your application, LOSFA will forward to you a Service Level Agreement for your signature. You must return this signed agreement to complete the application process and be considered for an award. If you fail to return the service agreement within the time allotted, you will be disqualified and eliminated from consideration. If it is determined that you are one of the least able to pay in your region, LOSFA will contact you to request documentation to substantiate the statements made in your application. For each subsequent year you wish to apply, a separate application must be completed.
